Caregivers
- Receive a tax-free stipend ranging from up to $18,000 annually based on participant’s care needs.
- Enjoy two weeks of paid annual respite relief (time-off).
- Ongoing support and training from a nurse and social worker.
Participants Needing Care
- Live with a caregiver who assists with personal care and medical needs.
- Personalized care plan and ongoing support from a nurse and social worker.
Eligibility
- Able to meet daily care needs of the participant and program guidelines.
- May be family/friend of the participant, except a spouse or legal guardian.
- Lives in the same home as the participant.
Participants Needing Care
- 22yrs old and older
- Unable to live alone
- Chronic medical or psychiatric condition
- Require assistance with at least one of the following: Bathing, dressing, toileting, transferring between positions, ambulating, or eating
- Have MassHealth Standard or CommonHealth (Maybe a member of a Senior Care Options [SCO], OneCare plan, or PACE.

We Provide A Dedicated Team
to assist our members with booking appointments, transportation, refill medications, finding & arranging community programs to help member stay/improve current health. Provide education in members’ language on chronic conditions by a care team that can speak your language. NO MORE LANGUAGE BARRIERS
We Focus On The Mind & Body Connection
To educate members and caregivers about mental health conditions and teach simple tricks to improve mindfulness (meditation methods) to improve anxiety and depression. With over 14 years of community care our director creates a special plan of care for each member. We understand the difficulty of aging minds therefore our strategy is “small moments several times” we do not rush or give up on our seniors.
We Are The ONLY AFC Care
Coordination Agency that will take over the administrative, care coordination responsibilities of an AFC caregiver, allowing you more time to spend with your love ones.
We Believe Technology Holds
the power to end many problems in senior care. All our members while enrolled in our program will have a tablet with unlimited WIFI, they now can do telehealth appts, have interactions with our care team, attend group classes without having to leave home.
We Reward Our Members
who complete the “Healthy Habits” task during the first year of enrollment & receive $100 to spend or pick “Date Night” an all-expense pay date night for our senior and one guest.

Our office hours are:
Sunday: 10am-4pm
Monday: 10am-5pm Wednesday: 10am-5pm
Saturday: 10am-7pm
We are available 24/7 for our members via Chat or Call during office hours for immediate services. All calls outside those hours are answered by a Chat system monitored by live agents however non-urgent matters returned calls to the member within a 12-hour window. All life-threatening emergency members are to call 911
