Clare’s Geriatric Care Coordination Program was created to fix the challenges many families face with traditional healthcare—like staff shortages, complicated care systems, caregiver burnout, and poor management of chronic conditions. Our goal is to make care easier, more effective, and more supportive for seniors and their loved ones.

We also understand that many older adults struggle with memory loss and the negative effects of being socially isolated. That’s why our program focuses on reducing loneliness, making telehealth simple to use, and offering resources that are culturally appropriate and easy to understand—so every senior feels connected and cared for.

How We Support Our Members

At Clare, we go beyond providing care—we ensure every detail is managed so seniors and their families feel supported and connected. Our team works closely with members, caregivers, and healthcare providers to create a seamless care experience.

Enrollment and Setup

We make getting started simple and stress-free. Our staff assists with the enrollment process within a two-week timeline, sets up member portals, and ensures communication tools like iCloud, calendars, and reminders are ready to help with appointments and medications. We also create secure group chats in Spruce to keep members, their care teams, and the Clare office connected.

Documentation

Clare takes regulatory compliance seriously. We maintain accurate progress notes, update electronic medical records, and review member charts to ensure proper documentation. Our team monitors incoming communications daily to provide timely and accurate responses.

Communication and Care Coordination

Clear, timely communication is at the heart of what we do. We handle correspondence through trusted platforms such as Spruce, Google Voice, and Tandem. Our team coordinates medical appointments, prescriptions, medical supplies, and even transportation bookings. We also connect members to community resources and state assistance programs to support their wellbeing.

Internal Support and Development

We believe in supporting not only our members but also our team. Staff attend regular meetings, assist field teams, and help maintain a professional and welcoming office environment. We provide translation services when needed, promote community events, and support caregivers with enrollment, HR questions, and training.

Financial and Administrative Support

We manage the details behind the scenes so families can focus on care. This includes billing, prior authorizations, insurance contracts, and required PT1 approvals. Our staff also keeps careful records to ensure compliance and transparency.

Community Outreach

To better serve our community, Clare actively maintains a strong online presence across social media, SEO, and digital platforms. These efforts allow us to connect with more families who may benefit from our services, while also keeping current members informed, engaged, and supported with important updates and resources. By combining outreach with education, we ensure that both seniors and caregivers know about the care and programs available to them.
