A. Who is covered

This Policy applies to all clients, caregivers, applicants, staff, contractors, and visitors engaging with Clare’s services, including Adult Foster Care (AFC), Group Adult Foster Care (GAFC), private-pay services, telehealth, and all program activities.

B. Our commitment

We will provide equal access to services and benefits without discrimination, including in:

  • Eligibility, enrollment, and service delivery (AFC/GAFC and private-pay).
  • Clinical interactions, care coordination, and case management.
  • Communications, digital content, and physical access to locations.

C. Free aids & services for accessibility

Clare provides, free of charge, reasonable aids and services to help people with disabilities communicate effectively, such as:

  • Qualified sign language interpreters.
  • Written information in alternative formats (large print, audio, accessible electronic, Braille, as reasonably available).

D. Free language assistance

If your primary language is not English, you can get help in your language at no cost:

  • Qualified medical interpreters (in-person, video, or phone).

E. How to file a grievance with Clare

If you believe Clare has failed to provide the services above or discriminated in any way, you may file a grievance. Include your name, how to contact you, what happened, when, and any supporting details.

We will confirm receipt, investigate promptly and impartially, and issue a written response. You may have a representative assist you. You can also request auxiliary aids or language help for the grievance process at no cost.

F. How to file a civil rights complaint with HHS OCR

You can also file a complaint directly with the U.S. Department of Health & Human Services, Office for Civil Rights (OCR) within 180 days of the alleged discrimination:

Filing a grievance with Clare does not prevent you from filing with OCR (and vice versa). Clare will not retaliate against you for filing a grievance or complaint, or for participating in an investigation.
