Adult Foster Care (AFC)

Adult Foster Care (AFC) allows a senior or person with a disability to live in a private home with a dedicated, live-in caregiver (often a friend or family member). MassHealth provides the caregiver with a tax-free stipend and professional nursing oversight to ensure the participant receives daily help with personal care in a family-style setting.

Group Adult Foster Care (GAFC)

Group Adult Foster Care (GAFC) provides daily personal care and medication reminders to individuals living in assisted living or subsidized housing. Rather than a live-in relative, care is delivered by visiting agency professionals. While MassHealth covers the cost of these medical and supportive services, the participant is responsible for their own rent and food.

Workforce Development

Clare Senior Care’s apprenticeship addresses Massachusetts’ 15,000-job healthcare shortage by training Home Health Aides through an “earn-while-you-learn” model. Partnering with CTIS and MassTech, the program combines hands-on care with digital literacy to safely transition patients from hospitals to home.

How to Become an AFC Caregiver in Massachusetts

Geriatric Care Management

Our Geriatric Care Management service provides expert guidance to help seniors and their families navigate today’s complex healthcare system. For $1,500 per month, clients receive a dedicated care manager, a personal iPad for easy communication, and monthly check-ins to ensure ongoing support. We coordinate medical appointments, manage prescriptions, and connect members with essential services. This professional oversight gives families peace of mind, knowing their loved ones receive comprehensive, well-organized care. Additional private care services are available if needed.
