Step 2: Home Safety Inspection

As part of the AFC enrollment process, Clare Senior Care staff will schedule a day to visit the member’s home. This inspection is required to ensure the environment is safe and meets program standards. Families do not need to prepare paperwork—our team will guide the process.

During the in-home assessment, Clare Senior Care staff provide a complimentary fall risk check-up. This quick review helps identify safety hazards and offers simple recommendations to keep the member’s home safer and more accessible.

During these visits, our case managers check health, review care plans, and support both members and caregivers with guidance and resources. Step 3: What is Clare Senior Care AFC?

Clare Senior Care helps seniors stay safe at home through MassHealth-funded programs like Adult Foster Care (AFC) and Group Adult Foster Care (GAFC), as well as private home health services. Our mission is to provide compassionate, culturally inclusive care that supports both seniors and their caregivers every step of the way. We help you handle the details so your loved one gets the care they deserve:

  • Transportation – MassHealth covers all medical rides. We take care of approval and booking—just give us 3 days’ notice.

  • Medication Refills – Tell us what’s needed, and we’ll coordinate directly with the pharmacy.

  • Referrals & Medical Supplies – We can place and manage orders for you.

  • Dental, Vision & Hearing100% covered for annual exams—essential for preventing isolation, depression, and dementia in seniors.

About Us Our Staff

Adult Foster Care (AFC) is a MassHealth program that allows seniors and adults with disabilities to live safely at home with the support of a trained caregiver. Caregivers provide daily help with activities like bathing, dressing, and meals, while receiving a non-taxable stipend and ongoing support from Clare Senior Care’s nurse and case manager team.

What Is AFC ?

Activities of Daily Living (ADLs) include basic self-care tasks like bathing, dressing, eating, and walking. Instrumental Activities of Daily Living (IADLs) are tasks that support independent living, such as cooking, shopping, managing money, and taking medications. MassHealth uses ADLs and IADLs to decide eligibility for programs like AFC and GAFC.

MassHealth’s Adult Foster Care (AFC) program has two levels of support.

  • AFC Level 1 – For members needing help with 1–2 ADLs or reminders for daily tasks. Earn $350 every 2 weeks ($25/day) and receive 1 monthly CareTeam visit.
  • AFC Level 2 – For members needing help with 3 or more ADLs, or 2 ADLs plus behavioral support. Earn $750 every 2 weeks ($53.57/day) and receive 2 monthly CareTeam visits.

As part of enrollment, caregivers complete HIPAA training to understand how to protect a member’s personal and medical information. This training ensures all care is delivered with privacy, security, and respect for confidentiality.

The Participant Agreement outlines the rights and responsibilities of joining Clare Senior Care’s AFC program. By signing, members and caregivers confirm they understand the program rules, services provided, and expectations for maintaining a safe and supportive home environment.

Clare Senior Care provides eligible members with an iPad on loan to support communication, training, and care coordination. This device helps families stay connected with our team, complete forms, and access telehealth or educational resources during the AFC program.

Clare Senior Care may provide a cell phone to support communication between caregivers, members, and our care team. This agreement explains proper use, care, and responsibility for the device to ensure reliable contact in emergencies and during daily care.

Clare Senior Care offers certificate-based training to prepare caregivers with the skills needed for safe, effective, and compassionate care. Completing this training provides caregivers with official recognition of their learning and ensures they meet MassHealth program standards.

If you prefer to learn at home instead of coming into the office, Clare offers the Personal and Home Care Aide State Training (PHCAST) online at no cost. This free program helps caregivers build skills and meet training requirements from the comfort of home.

Clare Senior Care offers in-person training sessions to prepare caregivers with the skills and confidence needed for daily care. These hands-on classes cover essential topics like ADL support, documentation, safety, and emergency planning—ensuring caregivers are fully supported and ready to provide quality care at home.

QARI Counseling Center provides professional mental health support with Mandarin-speaking, master-level clinicians. The team is trained in evidence-based therapies including Cognitive Behavioral Therapy, Narrative Therapy, Solution-Focused Therapy, Expressive Therapy, and the AWARE Protocol—specialized for Asian women’s resilience and empowerment.
