Adult Foster Care (AFC) Program
The Adult Foster Care (AFC) program is designed for MassHealth members who cannot live alone safely due to medical, physical, or cognitive challenges.
Through our Elite Care Model (ECM), we provide families with financial stability, professional clinical oversight, and extensive administrative support to ensure your loved one thrives at home.

Insurance & Paperwork
We manage and submit all MassHealth applications and handle all annual benefit renewals for you.
Medical Logistics
We find specialists within your network and book doctor appointments on your behalf.
Transportation
Need a ride to the clinic? We coordinate and book transportation for all medical visits.
Custom Coordination
We arrange activities and services based specifically on the requests of the family or caregiver.
Multilingual Support: Resources and assistance available in English, Chinese, and Vietnamese.
2. Financial Compensation
Caregivers receive a tax-free stipend and monthly bonuses to support the essential daily care they provide. Compensation is determined by the member’s clinical eligibility level.

Important: All stipends provided through the AFC program are currently non-taxable income under IRS Notice 2014-7, ensuring you keep the full amount for your household needs.
3. Clinical Oversight & Care Team Visits
To keep your loved one safe and healthy, our Registered Nurse (RN) and Care Manager (CM) provide professional home visits for both Level 1 and Level 2 care:

“Rotating Support” The Nurse visits one month, and the Care Manager visits the next. You see a professional once every month, rotating between clinical and management expertise. |
