The GAFC program provides essential daily assistance for seniors and individuals with disabilities living in subsidized housing or assisted living communities.

Insurance & Paperwork
We manage and submit all MassHealth applications and handle all annual benefit renewals for you.

We find specialists within your network and book doctor appointments on your behalf.

Need a ride to the clinic? We coordinate and book transportation for all medical visits.
Custom Coordination
We arrange activities and services based specifically on the requests of the family or caregiver.
Multilingual Support: Resources and assistance available in English, Chinese, and Vietnamese.
2. Home Support & Wellness Services
The GAFC program provides consistent professional oversight and household assistance to ensure members can live independently with peace of mind.

| Daily Care Visits | A daily visit from a Home Health Aide (HHA) to assist you with Activity of Daily Living (ADL) to ensure your essential daily needs are met. |
| Monthly Deep Cleaning | A comprehensive cleaning of the living space once per month to maintain a healthy and sanitary home environment. |
| Clinical Oversight | Ongoing health monitoring with monthly on-site visits from either a Registered Nurse and a Care Manager. |
Note: Daily visits are designed to provide professional assistance to ensure your ADL needs are met and your overall stability is maintained within the home.
3. Health Monitoring & Technology Devices
We provide specialized medical equipment and digital tools to monitor your vitals and keep you connected with your care team.
Vital Sign Monitoring
Used to track heart health and ensure medications are working correctly by measuring your blood pressure regularly.
A simple device that clips to your finger to check your blood oxygen levels and pulse, ensuring your respiratory system is stable.
Communication Tools
Used for telehealth visits, digital paperwork, and health resources.
