As part of the AFC enrollment process, Clare Senior Care staff will schedule a day to visit the member’s home. This inspection is required to ensure the environment is safe and meets program standards. Families do not need to prepare paperwork—our team will guide the process.

Caregivers provide daily help with activities like bathing, dressing, and meals, while receiving a non-taxable stipend and ongoing support from Clare Senior Care’s nurse and case manager team.

As part of enrollment, we ask every family to provide an emergency contact. This ensures Clare Senior Care can quickly reach someone you trust if urgent situations arise, keeping the member safe and supported at all times.

4. Participant Agreement
The Participant Agreement outlines the rights and responsibilities of joining Clare Senior Care’s AFC program. By signing, members and caregivers confirm they understand the program rules, services provided, and expectations for maintaining a safe and supportive home environment.

This agreement ensures that every member has a trained caregiver available at all times. If the primary caregiver cannot provide care, a backup caregiver is designated to step in. By signing, families confirm they understand the plan for safe and continuous support.

Clare Senior Care provides eligible members with an iPad on loan to support communication, training, and care coordination. This device helps families stay connected with our team, complete forms, and access telehealth or educational resources during the AFC program.
