Caring With Purpose at Harbor Point
Clare Senior Care’s community outreach team recently brought care and connection to Harbor Point on the Bay Apartments in Massachusetts, where our head nurse, Ms. Thai Nguyen, led a free Blood Pressure Clinic for senior residents. The event aimed to raise health awareness, encourage regular monitoring, and provide personalized advice for residents seeking to improve heart health and daily wellness.
A Meaningful Morning of Health and Connection
During the session, Nurse Thai Nguyen met with residents for an informal health talk focused on the importance of blood pressure control, healthy habits, and preventive care. Residents were invited to have their blood pressure checked, learn how to track their readings, and receive one-on-one tips for diet, stress management, and medication safety.
Beyond medical guidance, the event also created an opportunity for open dialogue. Many residents shared questions about MassHealth programs—especially the Group Adult Foster Care (GAFC) option, which provides daily personal care for eligible adults in supportive housing. Clare’s staff helped explain how GAFC works, who qualifies, and how it supports independence while ensuring consistent care at home or in the community.
Continuing the Mission
Clare Senior Care believes healthcare starts with trust, education, and presence. This event was part of our ongoing effort to make healthcare approachable and inclusive for every senior, in every neighborhood. Seeing the enthusiasm from Harbor Point residents, Clare hopes to host similar clinics every two weeks, providing continuous support and building a lasting relationship with the community.
